Alternate Examination (EX2)

Subsequent Examination 120 minutes (for those who have been away from the office for more than two years), includes:

• Re-Complete Medical History Evaluation,
• Complete Physical Examination,
• Re-Diet Evaluation,
• Urine Dipstick Test and Blood Sugar.

Subsequent Examination (EX3)

Subsequent Examination 90 minutes (for those who have been away from the office for more than a year), includes:

• Re-Complete Medical History,
• Physical Examination,
• Re-Diet Evaluation,
• Urine Dipstick Test and Blood Sugar.

Progress Check Examination (EX5)

Subsequent Examination 30 minutes (Current Patients) , includes:

• Re-Evaluation of last visit’s Symptoms,
• Re-examination of last visit’s symptoms,
• Check for any aggravations,
• Check for new symptoms,
• Adjust Remedies and diet according to improvement,
